Free support for groups, community support organisations, health and social care providers, social prescribers, and others to help people with their future care planning.

If you support people who are entering later life, dealing with serious health conditions, or making care choices for themselves or others, you know how challenging it can be for them to plan for the future. All they want is to focus on living their best life right now!

What if you could help them plan for tomorrow and live for today?

This is where we come in. My Future Care is a project of the not-for-profit social enterprise My Care Matters. We are dedicated to encouraging people to think about and make decisions much earlier than tends to be the case.

We have created the trusted My Future Care Handbook –  a user-friendly and jargon-free workbook which covers everything from writing a Will and putting a Power of Attorney in place, to assessing care options and keeping fit. It really is a “one-stop-shop” for later-life planning.

And this is where you come in. We provide organisations and groups like yours with free and easy-to-implement training and ongoing support, structured around the My Future Care Handbook, to help you better meet the needs of the people you work with.

So, what does My Future Care Support include?

We will consult with you to tailor a support package that fits your needs, based on the following three steps.

1 - Training and/or awareness sessions according to your team’s needs

If you are new to future care planning, we will upskill your team (staff or volunteers) in two ninety-minute Training Sessions. We cover key future care planning topics and guidance on supporting people using the My Future Care Handbook.

If you already have future care planning knowledge we can arrange a shorter 45-minute Awareness Session to introduce the Handbook.

Training and Awareness Sessions are free, thanks to our funding partners.

2 - Printed copies of the My Future Care Handbook

We can supply you with copies of the My Future Care Handbook to give to your clients or group members. Thanks to our funding partners, we are able to supply these at a significantly discounted price. Ask us for further details.

3 - Ongoing support and future care planning resources

You become a My Future Care community member and receive ongoing support from our team and your peers. You’re invited to online meetups, join our online portal and receive regular email updates and resources.
