Supporting Communities in Norfolk to Plan for the Future

We offer fully funded training plus My Future Care Handbooks and ongoing support to organisations, community groups and professionals in Norfolk who want to help people plan ahead for later life and possible future care.

I’m interested in supporting people to plan for the future

Why this matters

If you support people who are entering later life, living with serious health conditions, or making care choices for themselves or others, you know how challenging it can be for them to plan for the future. Often, they just want to focus on living their best life right now.

What if you could help them plan for tomorrow and live for today?

That’s where we come in.

My Future Care is a project of the not-for-profit social enterprise My Care Matters.  We’re dedicated to helping people make informed choices and take practical steps earlier — with warmth, compassion and easy-to-use tools that make planning feel positive and achievable.

About the My Future Care Handbook

At the heart of our approach is the My Future Care Handbook — a friendly, jargon-free workbook covering everything from writing a Will and setting up a Power of Attorney to exploring care options, staying well and recording personal wishes.

Every Norfolk-based organisation that takes part in our funded training receives free/subsidised Handbooks to share with the people they support, helping them start practical, empowering conversations in their own communities.

The Handbook acts as a simple, structured guide — turning what can be a daunting topic into a positive, forward-looking conversation about what matters most.

What is the training about?

These sessions aren’t about turning you and your team into future care planning experts — they’re designed to give you the confidence and skills to start meaningful conversations and to support individuals in identifying and acting on their priorities.

We’ve trained a wide range of people, from volunteer community group organisers to seasoned palliative care nurses already helping people plan their future care.

The training helps participants to:

  • Understand the importance and benefits of advance care planning
  • Explore the challenges some individuals face when discussing their future
  • Gain practical knowledge of advance care planning tools and documentation
  • Learn key coaching techniques
  • Confidently support individuals in setting and achieving their goals for later life and future care planning, using the My Future Care Handbook as a conversation guide.

All we ask is that you:

  • Encourage the people you support to use the My Future Care Handbook after training

  • Share a little feedback to help us learn and improve

We can share lots of ideas and examples from other organisations across Norfolk as to how you might do this.

What impact is My Future Care having in Norfolk already?

Organisations across England are already using the My Future Care Handbook to open up meaningful conversations about later life and future planning. Here are just three examples:

“Using the Handbook has enabled carers and family to gain an understanding as to what to expect, to explore their own wishes and feelings and most importantly where to find information. These sessions have provided a safe space to explore future planning in an informal setting, promoting discussion and peer support.”
